✓ Immunization Record

     ► DTap (4-5 doses, last dose on or after 4th birthday and 6 months                  from previous doses) OR TD (if child is 7 years or older)

     ► TOPV (4-5 doses, last dose on or after 4th birthday and 6 months                from previous doses)

     ► MMR (two doses at least 4 weeks apart) OR MR (if child is over 7                years old) Hep B (#1 through #3)

 

✓ Result of a PPD (TB skin test)

    Latest skin test result not later than 1 year prior to registration for student       from the U.S. or its territories, OR 6 months for students entering from             non U.S. areas. 

*NOTE: If your child’s results are positive, you must get a clearance from the Dept. of Public Health’s CDC office in Mangilao before entrance to school.

 

✓ Physical Exam - Completed within 1 year of entry into any school.

 

✓ Official Birth Certificate (NOT a copy) OR passport

 

✓ Legal Documents - Guardians must provide a copy of the Court               Order signed by the judge or a Notarized Original of Guardianship papers.

 

✓ Mayor’s Verification of Residency

 

✓ Withdrawal or Transfer Form

*MUST include the following: Health Audit from prior school

    Most recent report card

    Meal application form
